Please read the following information before beginning the registration for our Black Tie Affair Celebrating

our Pastor, The Rev. Dr. Kevin D. Sturdivant's Fifth Anniversary! 

This Spectacular event is on Friday, May 17th at 7pm in our Multipurpose Center!

The evening includes great food, fellowship, live music, and entertainment!

Registration is required and the cost is $20 for Springfield members and $30 for non-members. 

Steps to secure your seats!

1) Complete the form below by entering your information and if applicable the information of your guest(s). Be sure to indicate if each person on the registration are Springfield members or not. Enter each attendee's information accurately and then click REGISTER. 

Please don't enter your name multiple times for your guests, we need names of all attendees.

2) After clicking register, you will be directed to another page. Click the link to Springfield’s Giving Portal and pay for everyone you are registering.

Note the following:

Please be prepared to submit the total amount before beginning the registration process.

Up to 7 additional guests can be registered in a form, securing a table of 8.

Payments are NON-REFUNDABLE AND NON-TRANSFERABLE

Registration is on a first-come basis.
